Coaches confirm that you hold a current Australian Sports Commission Diving Coaching Accreditation.

Asthma Medication Notification Form
For the Asthma Medication Notification Form you will need to complete and return it to Diving NSW. You will find the address on the Diving NSW website under 'Contact Us' located at the top of the page.
